🤔 About the Role:
This role is for an experienced Pharmacy Technician (PT) to remotely manage patients with long-term conditions in a Primary Care setting. You will run your own clinic from home and work with other members of our clinical team. You will report to our Pharmacy Technician Manager with Clinical Supervision from a Clinical Lead.

💻 What you'll be doing :
Clinical Responsibilities
- Recording of patients health data on Suvera Dashboard & Practice EHR
- Interpretation, Risk stratification, and Management of this data in line with standard operating procedures
- Delivering lifestyle advice to patients to help manage their Long Term Condition.
- Communicating with and assisting other members of the clinical team where appropriate
- Conduct comprehensive reviews of patient information using professional and clinical judgment to identify needs, address concerns, and ensure safe and effective care, referring patients to clinicians or other services as appropriate. Including but not limited to the following conditions: Non-diabetic Hyperglycaemia (NDH), Hypertension, Asthma, Hypercholesterolemia.
- Ensuring all necessary reviews/assessments are completed and accurately coding and marking patients as QOF-complete to support annual target achievements.
- Conduct structured medication reviews to ensure safe, effective, and appropriate use of medicines, identifying opportunities for optimisation, communicating need for change to Clinical Pharmacists
- Gain informed consent for medication changes, provide patient education and counselling on prescribed medicines, including advice on adherence and managing side effects.
- Support ongoing monitoring of patients Long Term Condition, ensuring appropriate tests are ordered and completed by patients.
- Identifying and resolving medicine related queries, ensuring safe prescribing practice and adherence to NICE guidelines.
- Complete clinical tasks independently and asynchronously, ensuring thorough assessment and appropriate action, while escalating complex cases or concerns to the relevant clinician when required.
- Process medicine requests, ensuring accuracy, appropriate monitoring, and alignment with formulary and prescribing guidelines.
- Collaborate with Care team colleagues to provide seamless patient care.
Governance and Patient Safety
- Participation in Quality Management Processes including audit
- Timely referral of complex cases to appropriate clinician
- Regularly review and assess the quality of your own calls, identifying areas for improvement and ensuring high standards of patient interaction.
- Maintaining at all times a regard for patient privacy, safety and dignity
- Efficiently manage tasks by capturing accurate data in real-time while engaging with patients during calls. Documentation on Suvera Dashboard and GP partner systems.
- Being proactively involved in Quality Outcomes Framework (QOF) call and recall
- Liaising with and assisting members of the operations/administrative team
- Supporting and participating in shared learning within Suvera and our GP Partners
- Assist in the development and improvement of the Suvera’s systems
- Undertake other administrative duties as required, effectively managing time and prioritising tasks to balance clinical responsibilities and support service delivery.
- Keep up to date with all relevant organisational and role specific training as required (this includes mandatory and other training)
- Attend all meetings within the organisation and external as agreed with your line manager
- Attend regular multi-disciplinary meetings organised by the Care Team in order to discuss and learn from recent significant events relating to clinical practice occurring within Suvera .
Training / Self-Development
The Pharmacy Technician will participate in any training programme implemented by Suvera as part of this employment, such training to include:
- Participation in an annual Suvera appraisal , including taking responsibility for maintaining a record of own personal and/or professional development
- Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.
👀 We're looking for someone who:
Qualifications
- Currently working as a Registered Pharmacy Technician within NHS and have more than 12 months experience in this role
- Completed a two-year accredited pharmacy technician course involving a mix of practical work experience and study via GPhC-accredited courses, GPhC-recognised qualifications or a GPhC approved apprenticeship pathways
- Act at all times within the boundaries of the Professional Code of Ethics and Conduct
- Evidence of continuing professional development (CPD) relevant to the role
- Hold a recognised Medicines Optimisation Qualification e.g MOPS
Experience
- Acts at all times within the boundaries Professional Conduct
- Has excellent communication skills, both written and spoken
- Feels confident communicating with patients via their preferred communication channel which could include email, messaging, telephone, or video calls
- IT literate, and able to work with a variety of software systems, including but not limited to Emis Web, SystmOne, Microsoft Office.
- Able to follow Standard Operating Procedures and policies at all times
- Experience in reviewing patients' blood results to inform clinical decision-making, ensuring appropriate follow-up and care aligned with best practices and clinical guidelines.
- Underpinning knowledge of Long Term Conditions that are managed in Primary Care.
- Competent in assessing clinical information and making informed decisions, escalating appropriately when required.
- Minimum of 12 months’ experience in patient-facing roles, demonstrating effective communication and problem-solving skills
- Previous experience working for a digital healthcare provider
