The heart of this role is clinical care. You’ll use your expertise to support patients with long-term conditions, ensuring safe and effective treatment while helping them make sustainable improvements to their health.
Here’s a high-level overview:
- Clinical care first – reviewing patient information and clinical letters, reconciling medicines, recording health data accurately in Suvera’s Dashboard and partner GP Practice systems, and supporting patients with conditions such as NDH, hypertension, asthma, and hypercholesterolaemia.
- Risk & safety – interpreting and stratifying clinical risk data, accuracy checking prescriptions, monitoring treatment in line with SOPs and guidelines, and escalating complex cases promptly to the right clinician.
- Patient support – providing lifestyle advice (e.g. diet, exercise, smoking, alcohol), counselling patients on prescribed medicines (including adherence, managing side effects, and informed consent for medication changes), and ensuring patients understand their treatment.
- Collaboration – working closely with Suvera’s multidisciplinary team and GP partners to deliver safe, joined-up, patient-centred care, and contributing to clinical governance, audits, and quality improvement.
- Growth & learning – engaging with training, mentoring where appropriate, and contributing to a culture of shared learning and continuous improvement.
